Evaluation Forms
It is normally the candidate's responsibility to ask each of their chosen referees to complete online the appropriate Search Associates evaluation form for them. However, if a candidate prefers to ask David Cope to approach her or his referees direct, that is perfectly acceptable.
Your chosen supervisor and parent referees are required to fill in their Search Associates evaluation forms electronically, using the online forms which can be obtained by submitting one of the forms from the drop-down list below.
Important Note for Candidates: The online evaluation forms can only be completed by your referees AFTER you have begun to submit your own online application to David Cope.
Guidelines for Referees - How To Complete An Electronic Evaluation Form
To complete an online confidential evaluation form, please follow this simple 5-step process:
Step 1: Select from the drop-down list below the type of evaluation form you wish to submit, and then type in the last name of the candidate you wish to evaluate in the space provided.
Step 2: Select the correct candidate from the resulting list which will pop up.
Step 3: Complete the resulting evaluation form online.
Step 4: Submit the completed evaluation form to Search Associates by clicking the "Submit" button at the bottom of the completed form.
Step 5: NOTE : AFTER YOU SUBMIT THE COMPLETED FORM, A FINAL VERSION OF THIS EVALUATION FORM WILL POP UP. IT IS VERY IMPORTANT that you print and sign this final page and post (by airmail if from outside the UK) the signed copy to the Associate listed at the bottom of that page. Thank you for your help.
Referees, please remember to print out the evaluation form you have submitted electronically and to send a signed copy of the evaluation to David Cope by post (by airmail if from outside the UK).
Candidates, if you are approaching your referees yourself, please remind each of them to print out the form they have submitted electronically and to send a signed copy of their evaluation to David Cope by post (by airmail if from outside the UK). If it is possible for you to do so, giving each of your referees a correctly stamped envelope (an airmail envelope if the referee is outside the UK) that is addressed to:

Exceptionally, and only in cases where a referee has no internet access available and is therefore unable to submit their evaluation electronically, please write to David Cope to request a PDF version of the evaluation form for the referee concerned to print out, complete and submit by post (by airmail if from outside the UK). We hope such cases will be extremely rare, mainly because recruiters far prefer to be able to access candidate references online.
